About The Position

Sentara Health is seeking an RN for its Observation/Transitional Care Unit: The Obs/Transitional Care Unit is a 9-bed short stay unit used to care for med-surg patients who will be discharged within 24-48 hours. The position is full-time on the night shift. The RN will assess, plan, develop, implement, and evaluate plans of care for specific patient populations, coordinating with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfer.

Requirements

• RN Licensure - Virginia Required
• BSN, RN-Associate's Degree OR RN-Diploma Required
• Two years of nursing experience

Nice To Haves

• Previous acute care experience

Responsibilities

• Assess, plan, develop, implement, and evaluate plans of care for specific patient populations.
• Coordinate and collaborate with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfer.
• Communicate capacity management plan to nursing units, leadership team, and physicians.
• Possess clinical knowledge and skills to meet standards of care as required by patient populations.
