Requirements

• Current Arizona RN license.
• CPR Certification.
• Basic Life Support (BLS) Certification.
• CPI Certification required prior to patient interaction.
• Associate Degree in Nursing (ADN) or Diploma nurse is required; BSN preferred.
• Experience in Behavioral health preferred.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Experience in discharge planning.
• Strong communication skills.

Responsibilities

• Assess patient physical, psychological, developmental, cultural, and discharge planning needs.
• Formulate a plan of care utilizing assessment data and input from patients, families, and the health team.
• Implement the plan of care through direct patient care and coordination of the healthcare team.
• Evaluate the patient's response to care and revise the plan of care as needed.
• Document assessment, planning, implementation, and evaluation in the patient record accurately and timely.
• Provide care based on the best evidence available and participate in research activities within clinical practice.
• Perform other functions as assigned by a registered nurse or the Director of Nursing.
