About The Position

The HVI Interventional Cardiology Nurse Navigator is responsible for coordinating the individualized patient and family centered care of cardiac patients throughout the care continuum. They will work closely with physicians in the Cardiac Cath lab to coordinate the patient’s care plan, serving as a member of the multidisciplinary team, functioning as a patient advocate, educator, and liaison between the patient and all other appropriate services after discharge. The Nurse Navigator will coordinate and work collaboratively to ensure efficient transition of care for all referred cardiac patients. The initial patient population will be confined to Hartford Hospital Medical Group Interventional Cardiology elective patients admitted to JB2 with plans to extend role as needed. The role will also include collaboration with the office staff in Medical Office Building to identify high risk patients\: such as caregiver issues, compliance problems, high levels of anxiety or other special needs related to the procedure. This role will work closely with the Cath Lab Manager to initiate collaboration with referring physicians and their office staff.

Requirements

  • Bachelor’s Degree in Nursing Required.
  • Experience as a registered nurse in Acute care strongly preferred.
  • At least 2 years in cardiology Cath Lab or EP preferred.
  • Must be self-directed and able to function independently as well as maintaining an effective presence in a team setting.
  • Effective leadership skills with the ability to clearly communicate with tact and confidentiality.
  • Ability to adapt and be flexible to the needs of the Heart and Vascular Institute as this role evolves.
  • Ability to utilize advanced critical thinking and judgment to make safe recommendations.

  • Master’s Degree/CNL preferred
