You’ll play an important part in ensuring each patient receives safe, individualized care while contributing to a culture focused on collaboration, learning, and excellence. Responsibilities

• Support patient care planning and delivery in collaboration with the nursing team
• Monitor, document, and communicate patient status and changes in condition
• Assist in developing and carrying out individualized care plans
• Provide education and support to patients and families
• Delegate appropriately and collaborate with LNAs/LPNs and multidisciplinary teams
• Maintain accurate, timely documentation and stay current with clinical knowledge
• Use sound judgment to prioritize care and respond to patient needs
• Contribute to a positive, organized, and patient-centered care environment
