About The Position

The Lung Cancer Screening Clinical Coordinator Registered Nurse (RN) is responsible for managing the nursing process, which includes assessing, planning, implementing, and evaluating patient care. This role focuses on patient education, self-management, and ensuring customer satisfaction while coordinating care across multiple providers and services within the Veterans Affairs system. The RN will demonstrate leadership and creative approaches to complex patient care, ensuring continuity of care for veterans and their families.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the U.S.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Proficiency in basic written and spoken English.

Nice To Haves

  • Previous Oncology and/or Case Management/Care Coordination Experience.

Responsibilities

  • Execute position responsibilities demonstrating expert leadership and experience in complex patient management.
  • Provide patient management, care coordination, and disposition planning for patients.
  • Assist veterans, family members, and caregivers in accessing appropriate options and services for their healthcare needs.
  • Coordinate care with multiple providers across various levels and sites of care.
  • Manage consults for potential lung cancer screening enrollment by performing chart reviews to verify eligibility and appropriateness.
  • Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
  • Communicate regularly with the interdisciplinary team and provide necessary reports.
  • Monitor and track abnormal images/lung nodules, ensuring providers are alerted to abnormal findings when necessary.
  • Collect, analyze, and report program evaluation data to assess the impact of lung cancer screening and implement evidence-based improvements.
  • Perform other duties as assigned.
