About The Position

The Registered Nurse, LIFE promotes and maintains health of participants in the community through teaching, counseling, and appropriate preventive and rehabilitative services. Responsible for assessment of health needs and contributing to the plan of care, initial implementation of nursing care plans, and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. Supervises medical assistants and LPNs in the clinic, to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants. Additionally, provides nursing support to the LIFE primary care staff. Demonstrates knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants according to assigned responsibilities and LIFE Program standards.

Requirements

  • Graduate from an accredited school of nursing, college or university required.
  • Licensure and current registration as a Registered Professional Nurse in the State of New York.
  • One year of experience working with the frail elderly required.
  • Minimum one (1) year relevant experience required.
  • Must possess a valid driver's license and have a car available for work use.
  • Strong written & verbal communication skills.
  • Computer skills required, i.e., software appropriate to position, word, excel.
  • Strong assessment skills required.

Nice To Haves

  • BSN preferred.
  • BLS certification preferred.
  • Knowledge of rules and regulations governing Licensed Home Health Agencies preferred.
  • Home Care experience preferred.
  • Acute medical surgical experience preferred.
  • Experience with patient assessment instruments preferred.
