About The Position

The Registered Nurse Level II at Prevea Health plays a vital role in delivering patient-centered care within a clinic team. This position involves providing direct patient care, educating patients and their families, and collaborating with healthcare professionals to ensure optimal patient outcomes. The RN Level II is responsible for conducting nursing assessments, administering medications, and maintaining patient records, all while adhering to the standards set by the Wisconsin Board of Nursing and Prevea's policies.

Requirements

  • Graduation from an accredited and approved Nursing program is required.
  • Bachelor's Degree in Nursing is preferred.
  • Current and valid Registered Nurse license with the state of Wisconsin is required upon hire.
  • CPR certification - Basic Life Support for Healthcare Providers certification from an approved program is required upon hire.

Responsibilities

  • Provides direct patient care including rooming patients and obtaining medical information.
  • Performs comprehensive initial patient assessments and reassessments pertinent to specialty area of practice.
  • Administers medications and monitors for reactions or side effects.
  • Maintains patient charts and records.
  • Follows the nursing process of assessment, planning, intervention, and evaluation.
  • Communicates information with patients, physicians, and staff to ensure quality and continuity of care.
  • Assists physicians or advanced practice providers with procedures and ensures their instructions are carried out.
  • Triages patient, family, and external provider concerns and inquiries.
  • Manages patient health concerns through telephone triage and facilitates access to appropriate care.
  • Refers patients to healthcare providers, services, and community resources as needed.
