About The Position

Charter Oak Health Center is seeking a Registered Nurse (RN) and/or Licensed Practical Nurse (LPN) with Internal Medicine experience to join our team. In this role you will be responsible for providing comprehensive patient care, coordinates with healthcare teams, and ensures the delivery of high-quality medical services. This role involves assessing patient needs, developing care plans, administering treatments, and educating patients and their families.

Requirements

Bachelor's degree in nursing (BSN) preferred.
Bi-lingual preferred.
Active Registered Nurse (RN) or LPN license in the State of CT.
Minimum of one-year experience in a clinical setting is preferred.
Strong clinical and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work in a fast-paced environment and handle stressful situations.
Proficiency in using electronic health records (EHR) systems (knowledge of EPIC and ECW preferred).
Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) is often required.

Responsibilities

Assess and monitor patients' health conditions, administer medications, and perform treatments.
Develop and implement individualized care plans based on patient assessments.
Work with physicians, specialists, and other healthcare professionals to provide coordinated care.
Educate patients and their families about health conditions, treatments, and preventive care.
Maintain accurate and detailed patient records, including medical histories and treatment plans.
Provide immediate care in emergency situations and assist in stabilizing patients.
Ensure compliance with healthcare regulations and standards.
Perform other work-related duties as assigned.
Flexible hours as required as well as potential rotation on Saturdays.
