Shift: 7pm-7am Days: Vary Position Summary: This position will work under the direction of a registered nurse while assisting patients account to care plans and department policy. This role ensures a comprehensive team approach in the care of patients and serves as liaison between nursing staff and patient in the performance of care plans.

Requirements

  • Must possess a current Arizona RN license.
  • BLS certification is required.
  • Associate Degree in Nursing (ADN) or Diploma nurse is required.
  • Two years behavioral health experience required.
  • Technical aptitude.
  • Strong communication skills (written and verbal).
  • Ethical Conduct.
  • Ability to perform tasks independently.

Nice To Haves

  • BSN is preferred.
  • Prior Intake experience is a plus

Responsibilities

  • Assesses patient physical, psychological, developmental, cultural and discharge planning needs.
  • Reviews patient history with patient/family and assures completion within appropriate timeframe.
  • Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers.
  • Completes assessment and reassessments according to patient need and as outlined in policy.
  • Formulates a plan of care utilizing assessment data and patient, family and health team input.
  • Initiates a plan of care based on patient need.
  • Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team.
  • Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements.
  • Evaluates the patient's response to care and revises the plan of care as needed.
  • Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members.
  • Documents assessment, planning, implementation and evaluation in the patient record.
  • Documentation is legible, timely and in accordance with policy.
  • Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment.
  • Notes physician orders accurately and in a timely manner.
  • Provides care based on the best evidence available and may participate in research activities within clinical practice.
  • Perform other functions as assigned by the Director of Intake or the Director of Nursing.
  • Coordinates physician to physician acceptance calls, participates in and documents acceptance calls
  • Assists with determining patient benefits/eligibility
  • Assists the team in proactively managing requests for admission and facilitates throughput of patients
  • Accurately collects data required for intake of patients from a variety of locations
