About The Position

The Registered Nurse (RN) Intake - Per Diem is a registered professional nurse responsible for assessing and evaluating patients being admitted to the hospital. This role involves taking physician orders and completing medication reconciliation with the physician, adhering to nursing processes and standards of care. The nurse must possess strong clinical skills and the ability to work independently with minimal supervision. The position requires working with children, adolescents, adults, and senior populations, necessitating familiarity with common diagnoses and specific assessment and interviewing skills for each group. Hospital for Behavioral Medicine (HBM) is a state-of-the-art 120-bed inpatient facility in Worcester, MA, dedicated to mental health treatment. In collaboration with UMass Memorial Health Care, HBM provides exceptional therapeutic care, prioritizing patient comfort and safety. Beyond inpatient services, the facility offers an outpatient partial hospitalization program, ensuring continuous support. Requirements

  • Graduate from an accredited RN program.
  • Currently licensed to practice by the Massachusetts State Board of Nursing.
  • Must pass a medication administration exam before administering medications.
  • CPR certification.
  • Successful completion of Management of Aggressive Behavior including restraint and seclusion policies, within 30 days of employment, and prior to assisting in restraining procedures.
  • Must have a working knowledge of the DSM-X.
  • Must have a working knowledge of the Mental Status Exam.
  • Must have a working knowledge of the Addiction and Recovery field.

Nice To Haves

  • Bachelor's Degree preferred.
  • A minimum of three (3) years experience in a psychiatric facility preferred.

Responsibilities

  • Assess and evaluate patients being admitted to the hospital.
  • Take physician orders.
  • Complete medication reconciliation with the physician in accordance with the nursing process and the standards of care and practices.
  • Complete the Nursing Assessment documenting patient information, medications, medical history, vital signs, and skin assessment.
  • Work with children, adolescents, adults, and senior populations.
  • Be familiar with common diagnoses with these population groups and familiar with specific assessment and interviewing skills for each population group.
  • Call physician for admission and medication orders.
  • Complete Medication Reconciliation by patient/family/pharmacy verification.
  • Report any signs or symptoms of infection, illness, medical instability to physician after assessment.
  • Document patient information and nursing care in the provision of nursing services.
  • Document and reassess any significant changes in the patient's condition in compliance with facility policy.
  • Walk patient to unit and give report to the unit nurse.
