Requirements

• At least three (3) years of RN work experience is required.

Responsibilities

• Independently coordinating and managing daily operations of the infusion suite, including scheduling and patient flow.
• Administering and monitoring intravenous and subcutaneous infusions in accordance with provider orders, protocols, and evidence-based practice.
• Coordinating and delivering comprehensive infusion therapy services in home, suite, and/or other clinical settings.
• Managing referrals, verifying clinical and administrative requirements.
• Providing direct patient care, education, and case management.
• Collaborating with physicians, pharmacists, and interdisciplinary teams to ensure safe, effective, and patient-centered care throughout the continuum.
• Coordinating intake and referral processes, verifying demographics, clinical history, authorizations, and consents.
• Developing and implementing individualized care plans based on thorough patient assessments, including venous system evaluation.
• Preparing and administering infusion medications using aseptic technique; perform vascular access procedures and lab draws.
• Educating patients and caregivers on therapy protocols, equipment use, and vascular access care.
• Monitoring therapy progress, adjust care plans, and communicate with providers to ensure optimal outcomes.
• Coordinating care and communicate effectively with providers, pharmacists, internal departments, and external healthcare teams to ensure continuity and quality care.
• Maintaining accurate documentation in the EMR and ensure compliance with clinical protocols and accrediting standards.
• Responding to patient and caregiver concerns, escalating issues appropriately to ensure resolution.
• Participating in the development of infusion protocols, patient education materials, and quality improvement initiatives.
• Supporting 24/7 care coverage through participation in on-call rotations, including weekends and holidays.
• Collecting and analyzing clinical data to inform care decisions and contribute to the evaluation of advanced care models.
• Traveling to patient homes and clinical sites to deliver infusion therapy, provide infusion related patient care, and perform patient teaching/assessments.
• Providing clinical oversight and guidance to Licensed Practical Nurses, ensuring adherence to care standards and supporting professional development.
• Other duties as assigned.
