Department Specific Job Responsibilities: ONCOLOGY INFUSION Safely administer a variety of chemotherapy/immunotherapy treatments, supportive care therapy and transfusions. Educate patient/family on self-care and reasons to call the office while receiving treatment. Safely place and care for peripheral IV access and central venous access devices. Evaluate and assess for tolerance to therapy, including laboratory values and physical symptoms. Collaborate with Radiation therapy, Nutrition, Social Work and Physical Therapy. Collaborate with both BIDMC Boston and BID Needham and Atrius Oncology in the care of patients. Department Specific Job Responsibilities: Determines the urgency and disposition for telephone triage requests and other patient calls requiring clinical expertise and decision making. Collects pertinent clinical data and applies nursing knowledge and evidence-based assessments to coordinate the delivery of care during telephone encounters, electronic communications and/or clinical visits. Facilitates the scheduling of urgent and/or routine appointments as needed. In collaboration with the healthcare team and PCP, participates in the coordination of care and on-going management of patients with stable chronic diseases by developing shared care plans, performing medication reconciliation, processing prescription renewals, performing symptom assessments, and coordinating VNA referrals. Collaborates with the LPN and the provider regarding ED visit follow-up and communicates with patients regarding symptom management and the coordination of additional services as needed. Promotes the health of patients through coordinated follow-up, supports improving health outcome efforts, education and self-management goals. Department Specific Job Responsibilities: Determines the urgency and disposition for telephone triage requests and other patient calls requiring clinical expertise and decision making. Participates in the coordination of care & on-going management of patients by developing shared care plans, performing medication reconciliation, completion of prior authorizations, processing prescription renewals within three days, performing symptom assessments, coordinating referrals and assisting prescribers in tracking and meeting timeframes for annual labs and AIMS. Communicates with patients and families (as consent allows), as well as other relevant stakeholders (ex. clinicians inside and outside BIDMC system), regarding symptom management and the coordination of additional services as needed. Facilitates the scheduling of urgent and/or routine appointments as needed. Promotes the health of patients through coordinated follow-up, supports improving health outcome efforts, education and self-management goals. Participates in clinical service meetings and quality improvement initiatives as an active member of the behavioral health team. Department Specific Job Responsibilities: PODIATRY Supports very busy podiatry clinic, providing nursing level support to patients, while supervising and supporting the role of medical assistants. Gathers information for H&P. Performs initial review of medication lists in OMR, performs med reconciliation when appropriate. Performs and documents evaluations and assessments of patient. Assists in ordering labs, x-rays, and diagnostic procedures. Works in collaboration with providers, to provides patient education on relevant to podiatric issues and wound care, as well as other diabetic health and nutritional issues. Assists in development of patient/family educational materials. Participates in patient rounds and case conferences. Preparation and application, under supervision when appropriate, of casting, boots, gauze, and other dressings, and orthotics. (Examples include: offloading pad and dressing, Unna compression wraps, total contact cast (TCC)). Dispense and measure surgical shoes, braces, CAM boots (or similar devices). Wound management: Performs removal of dressings, bandage changes, and basic wound cleaning/management, and application of dressings, including use of topical solutions or ointments. Assists providers with prep and application of skin substitutes and other tissue products. Provide support for common, routine procedures such as nail procedures, wart removal, wound debridement after completion of competency training, and removal of sutures and staples.
