About The Position

The Staff Nurse at Northwestern Medicine is responsible for providing individualized, age-specific care to patients throughout their continuum of care, from admission to discharge. This role requires adherence to the organization's mission, vision, values, and ethical standards while ensuring compliance with relevant policies and regulations. The nurse will assess patient needs, develop care plans, provide education, and collaborate with healthcare team members to achieve optimal patient outcomes.

Requirements

  • Bachelor of Science in Nursing (BSN)
  • Current State of Illinois Registered Professional Nurse
  • American Heart Association Basic Life Support (BLS)
  • Strong interpersonal skills
  • Ability to collaborate with others
  • Strong analytic abilities
  • Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role.

Responsibilities

  • Assess and record patient status and identify patient care needs upon admission/point of care entry.
  • Organize assessment health data to ensure accuracy, completeness, and accessibility.
  • Identify problems that require immediate attention and validate diagnoses with patients/families and healthcare team members.
  • Document diagnoses to facilitate the determination of expected outcomes and care plans.
  • Initiate and update the Problem List at the time of admission as needed.
  • Incorporate principles of age-specific care into the patient's plan of care.
  • Discuss and agree upon the individualized plan of care with the patient/family.
  • Begin discharge planning at the time of admission, consulting with patients/families and healthcare team members.
  • Identify patient teaching needs upon admission and throughout the hospital stay.
  • Arrange conferences with patients/families and healthcare team members to plan care when indicated.
  • Prioritize and organize evidence-based nursing interventions reflective of established standards of care.
  • Provide individualized patient and family education based on evidence-based practice and research guidelines.
  • Document all assessments, interventions, and patient responses.
  • Consistently review patient health data to keep the Plan of Care updated.
  • Ensure revisions to the plan of care are systematic and ongoing, driven by measurable goals.
  • Document patient progression toward identified outcomes and identify unmet outcomes for continuity of care.
  • Reassess nursing actions for effectiveness in achieving desired outcomes.
  • Communicate with patients/families and healthcare team members regarding patient care and nursing's role in care provision.
  • Collaborate with patients/families and healthcare team members regarding the patient's progress and plan of care.
  • Present relevant patient information at multidisciplinary meetings for peer feedback.
  • Make appropriate referrals as needed for continuity of care.
