About The Position

This position is for a Registered Nurse who is responsible for providing expert-level knowledge, skills, and abilities to provide, coordinate, and monitor nursing services to adults with physical disabilities as well as aging Delawareans to resolve both emergent and ongoing concerns referred to Adult Protective Services. The RN will also be responsible for DSAAPD community nursing as needed. The incumbent will be focused on interviewing and collaborating with individuals and their families, as well as advocating for the health, safety, welfare, or rights of individuals within DSAAPD and those referred to APS. The incumbent will assess progress, connect to resources, and refer as needed for additional follow-up services once the emergent clinical issues identified are addressed.

Requirements

  • At least two years' experience as a Registered Nurse OR a Bachelor's degree or higher in Nursing and at least one year experience as a Registered Nurse OR a Master's degree or higher in Nursing.
  • Possession of a Delaware Registered Nurse license OR multi-state compact license.
  • Applicants must be legally authorized to work in the United States.
  • The selected candidate must follow the federal conflict-of-interest requirements under 45 CFR 1321 for all duties related to the Older Americans Act Title III programs.
  • The candidate will be required to complete a Conflict-of-Interest Form to be provided at interview, disclose any actual or potential conflicts, and take part in annual reviews before beginning any Older Americans Act work.

Responsibilities

  • Provides or evaluates direct nursing care.
  • Develops, implements and/or reviews nursing care plans.
  • Provides health care instruction and guidance to individuals, families, community groups and/or nursing home facilities in the management of their health care needs.
  • Maintains an effective working relationship with other members of the treatment team.
  • Prepares and submits records, reports, evaluations and forms.
  • Participates in and make recommendations regarding quality assurance activities.
