About The Position

LifeLong Medical Care has an exciting opportunity for an experienced Registered Nurse III - HIV Case Manager to join our multidisciplinary team to provide patient care at our Alameda Health Centers . The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, an ability to work independently and an understanding of trauma informed care. Professional and empathetic communication is a key component of the position. Under the general supervision of the Center Director or Nurse Supervisor the Registered Nurse III - HIV Case Manager is responsible for independently providing patient triage in person and telephonically. The Registered Nurse III - HIV Case Manager will demonstrate that they are capable and competent in providing patient care, case management of complex patient needs and providing support for the providers, health centers, support staff and other site or program-based RNs. The Registered Nurse III will provide individualized case management, conduct patient triage, provide direct patient care, respond to patients concerns, provide education and consultation, review symptoms, provide vaccination counseling and education, review lab results, participating in the ongoing education of support staff and collaborating with medical providers to ensure optimal health of our patient populations. Requirements

  • Current, unencumbered license to practice as a Registered Nurse in California.
  • Ability to effectively coordinate care with triage assistants and other support staff, encourage and nurture development and growth, to build a strong and productive team.
  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
  • Ability to effectively present information to other team members.
  • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Work in a team-oriented environment with several professionals with different work styles and support needs.
  • Excellent interpersonal, verbal, and written skills.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and empathetic staff.
  • Associate or bachelor's degree in nursing.
  • Current and valid California Registered Nurse license.
  • Current BLS/CPR Certification.
  • Proficient in Microsoft office suite.
  • Electronic health records, EPIC preferred.

Responsibilities

  • Ability to work independently with high needs and high acuity patients.
  • Will act as a preceptor for less experienced nurses, ensuring that they are competent and capable of providing care and performing nursing tasks.
  • Ability to make quick decisions, multitask and handle stressful situations.
  • Possess strong written and verbal communication skills; ability to work as part of a team.
  • Has strong interpersonal skills, leads and provides patient care with compassion and empathy.
  • Trains, supervises, and evaluates the performance of assigned personnel, monitors staff for compliance with departmental policies, procedures, and regulatory requirements and compliance.
  • Provides direct professional nursing care to assigned patients.
  • Records patients medical histories, performs various diagnostic tests per provider or standing orders, administers medications, and develops patient care plans in conjunction with the primary care team.
  • Conducts co-visits and assists providers during examinations and procedures, operating and monitoring medical equipment as needed.
  • Participate in the coordinated care for patients seeking care via telephone and MyChart messages.
  • Follow up with patients and ensure communication with the primary care team.
  • Promotes independence by teaching patients and families to understand medical conditions, medications and self-care skills.
  • Coordinates with Care Transitions team for all hospitalized patients to ensures timely follow up and return to primary care.
  • Uses clinician-approved standardized procedures to order labs and preventive screenings, and to refill routine medications.
  • Coordinates care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met.
  • Acts as a resource for support staff.
  • Facilitates follow up care as needed.
  • Maintains absolute confidentiality of work-related issues, patient records, personnel records and County information; complies with the confidentiality standards of the Privacy Act of 1974, U.S.C. § 552A} as amended, and HIPAA policies and procedures.
