LifeLong Medical Care has an exciting opportunity for an experienced Registered Nurse III - HIV Case Manager to join our multidisciplinary team to provide patient care at our Alameda Health Centers . The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, an ability to work independently and an understanding of trauma informed care. Professional and empathetic communication is a key component of the position. Under the general supervision of the Center Director or Nurse Supervisor the Registered Nurse III - HIV Case Manager is responsible for independently providing patient triage in person and telephonically. The Registered Nurse III - HIV Case Manager will demonstrate that they are capable and competent in providing patient care, case management of complex patient needs and providing support for the providers, health centers, support staff and other site or program-based RNs. The Registered Nurse III will provide individualized case management, conduct patient triage, provide direct patient care, respond to patients concerns, provide education and consultation, review symptoms, provide vaccination counseling and education, review lab results, participating in the ongoing education of support staff and collaborating with medical providers to ensure optimal health of our patient populations.
