About The Position

In this position you will be the first point of contact for residents as they come into Delaware Veterans Home as well as assessing potential residents in the community and hospital setting. Your primary responsibilities include administrative tasks such as collecting clinical information and ensuring that all payor source paperwork is obtained. However, since you are a qualified nurse, we may expect you to provide patient care as well. While you will be working with a team of other medical professionals, you may administer medicine or take vitals when necessary. You will also educate and examine residents for potential issues before admissions and m ay travel for site visits as necessary. Experience working Skilled and Long-Term Care strongly preferred.

Requirements

  • At least two years' experience as a Registered Nurse OR a Bachelor's degree or higher in Nursing and at least one year experience as a Registered Nurse OR a Master's degree or higher in Nursing.
  • Possession of a Delaware Registered Nurse license OR multi-state compact license.
  • One year of experience conducting pre-admission assessments in a healthcare industry such as a hospital, surgical center, or long-term care facility.
  • All new hires are required to attend an in-person, statewide mandatory New Employee Orientation on their first day of employment. The session will be held at a designated location in either Kent or New Castle County.
  • Upon a conditional offer, you must undergo pre-employment drug testing as part of the hiring process.
  • A satisfactory criminal background check is required as a condition of hire.

Responsibilities

  • Provides or evaluates direct nursing care.
  • Develops, implements and/or reviews nursing care plans.
  • Provides health care instruction and guidance to individuals, families, community groups and/or nursing home facilities in the management of their health care needs.
  • Maintains an effective working relationship with other members of the treatment team.
  • Prepares and submits records, reports, evaluations and forms.
  • Participates in and make recommendations regarding quality assurance activities.
