Description Summary Under minimal supervision, provides nursing care for a group of patients assigned to the nurse based on matching the patients’ needs with the nurse’ competencies. Completes established competencies for the position within designated introductory period. Other related duties as assigned. Responsibilities Assesses human responses and adaptation to actual or potential health problems of the patient, family and/or significant other Institutes and initiates a plan for patient care based on the initial assessment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis. Recognizes changing patient condition and utilizes unit resources, chain of command and Rapid Response Team (RRT) in a timely and appropriate manner Documents all plans and care provided Identifies need for education of patient, family and/or significant other and provides education through individualized instruction, the use of written materials and/or other available resources Assigns nursing care to licensed staff, delegates nursing care to unlicensed staff and coordinates the flow of work Calculates and administers medications Coordinates and collaborates with multidisciplinary team members in the planning and management of patient care Participates in discharge planning; provides instruction to patient/family Maintains and promotes professional standards and development by participating in inservices, committees, staff meetings Serves as a role model for compassionate care and service excellence and commits to Studer Principles and “must haves” Facilitates patient movement within the department and through the facility, proactively managing patient admissions, discharges and transfers. Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required Provides every patient the defined standard of care (ie; core measures, chest pain protocol) Utilizes resources efficiently and effectively Maintains safe environment Addresses patient population—specific needs (clinical) Participates in Performance Improvement Activities Qualifications Education: MINIMUM EDUCATION: Graduate of accredited school of nursing. PREFERRED EDUCATION: Bachelor's Degree Experience: MINIMUM EXPERIENCE: None for Med/Surg and Behavioral Health 1 year in acute/chronic dialysis for Dialysis PREFERRED EXPERIENCE: 2 years in acute care for Med/Surg 2 years in psychiatric facility for Behavioral Health 2 years acute/chronic dialysis for Dialysis Certifications: REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License for Registered Nurse. For RN in Behavioral Health: Non-Violent Crisis Intervention must be obtained within 60 days of hire or transfer into department REQUIRED CCOURSES/COMPLETIONS (e.g., CPR): Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire. Dialysis Department: In addition to Basic Life Support, ACLS (AHA) must be completed within 60 days of hire or transfer into the unit. Behavioral Health Unit: Non-Violent Crisis Intervention for Behavioral Health must be obtained within 60 days of hire or transfer into role #LI-AP1
