As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Position Summary: Serves as a liaison between the organization and hospital staff, educating professional and non-professional team members on the organization’s services and programs. Facilitates effective communication and collaboration with physicians and interdisciplinary teams within the inpatient setting. Leads the development, implementation, monitoring, and supervision of strategic initiatives aimed at increasing referrals. Assists with referral coordination and ensures a seamless transition of care. Provides oversight and case management for hospitalized hospice patients. Reports directly to the Executive Director.
