The Hospice Registered Nurse (RN) is responsible for providing skilled nursing care to hospice patients in their place of residence and coordinating patient care with the Interdisciplinary Team (IDT), physicians, patients, and families. The RN performs comprehensive assessments, develops and manages hospice plans of care, and ensures quality patient care and regulatory compliance throughout the hospice care process. This role requires strong clinical assessment skills, care coordination experience, and the ability to work independently in a hospice field setting while collaborating with the interdisciplinary care team. Essential functions include performing initial and ongoing comprehensive nursing assessments, developing and updating individualized hospice plans of care, coordinating patient care with the IDT, physicians, patients, and families, educating patients and families on disease process, medications, symptom management, and end-of-life care, determining the scope and frequency of nursing services, supervising Hospice Aides and LVN/LPN staff, maintaining accurate clinical documentation, communicating changes in patient condition, ensuring compliance with Medicare hospice regulations, and participating in various meetings and activities to promote compassionate, patient-centered end-of-life care.
