Requirements

  • Graduate of an accredited school of nursing.
  • Current RN license in state of practice and CPR certification.
  • One to two (1–2) years of acute care experience required.
  • Strong clinical judgment, compassionate communication, and end-of-life care skills.
  • Valid driver’s license, reliable transportation, and required insurance coverage.

Nice To Haves

  • Hospice experience preferred.

Responsibilities

  • Perform comprehensive initial and ongoing assessments of patients and families to determine hospice care needs.
  • Develop, implement, and update individualized plans of care focused on comfort, symptom management, and quality of life.
  • Provide skilled nursing care, including medication administration, treatments, and palliative interventions as ordered.
  • Monitor changes in condition and communicate promptly with physicians to obtain updated orders.
  • Educate and support patients and families regarding disease progression, symptom management, and end-of-life care.
  • Serve as Case Manager when assigned, coordinating care for an interdisciplinary caseload.
  • Maintain accurate, timely documentation in the electronic medical record, including pain and symptom management outcomes.
  • Participate in interdisciplinary group (IDG) meetings and collaborate to ensure holistic patient care.
  • Participate in on-call rotation as needed and supervise ancillary staff as appropriate.
