About The Position

This position links patients and families with resources and services available in hospice facilities and through hospice at home. The role involves working with referral staff at discharge sites to ensure the best outcomes for patients and promote continuity of care, aligning with the agency's goals, strategies, mission, and objectives. The Registered Nurse is also responsible for determining patient eligibility and coordinating their scheduling into the hospice program. Goshen Hospice services Elkhart, LaGrange, Kosciusko and Noble Counties.

Requirements

  • Associates of Nursing (ASN)
  • Colleagues hired with an associate degree must enter a BSN program within 12 months of hire and earn their BSN from a NLN or CCNE accredited program within 5 years of beginning their BSN degree (exemption possible for those with an associate degree and 10 or more years of nursing experience hired effective February 2021)
  • Valid Indiana Registered Nurse license
  • American Heart Association CPR certification
  • 1 year of Medical Surgical nursing experience
  • Valid driver's license
  • Proof of auto insurance
  • Must be able to lift 50 pounds, assist with lifting 100 pounds
  • Good verbal and written communication skills

  • Bachelors of Nursing (BSN) preferred
