About The Position

The Registered Nurse (RN) position at UPMC Family Hospice and Palliative Care involves providing direct, one-on-one patient care in a hospice setting. The role focuses on supporting patients and their families through collaborative efforts with an interdisciplinary healthcare team. The RN will work primarily in the Pittsburgh East Suburb area, ensuring optimal patient satisfaction and care delivery while adhering to organizational standards and compliance training.

Requirements

  • Graduate of an accredited nursing program.
  • Nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
  • BSN preferred.
  • Ability to establish and maintain positive relationships with various stakeholders.
  • Good clinical judgment with critical thinking, analytical, and problem-solving abilities required.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.

Nice To Haves

  • Experience in hospice or palliative care settings.
  • Leadership experience in nursing.

Responsibilities

  • Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patients.
  • Completes an initial and comprehensive assessment of patients.
  • Initiates, coordinates, and ensures ongoing communication with the physician and other team members.
  • Provides direct patient care, evaluates outcomes, and adjusts nursing care processes as needed.
  • Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members.
  • Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed.
  • Completes all necessary documentation accurately and promptly in the electronic documentation system.
  • Exercises independent judgment in matters concerning emergent and non-emergent patient care needs.
  • Participates in weekend and on-call coverage rotation for the agency.
  • Demonstrates knowledge and understanding of the UPMC standard of care delivery.
