Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of Iowa.
• Ability to work well within a team environment and collaborate daily with nurses, social workers, chaplains, volunteers, and hospice aides.
• Capability to work closely with patients on a 1-1 level while receiving support from the patient care team.
• Ability to work independently on a day-to-day basis, managing activities such as phone, email, and written communication, and meetings with healthcare professionals.
• Strong time management skills to manage visits, documentation, and meetings, prioritizing and triaging multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, valid driver's license, and auto insurance to travel across the service area.

Responsibilities

• Providing care and services for a group of patients in various settings including private homes, Independent Living Facilities, and assisted living facilities.
• Acting as a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments and interventions.
• Working as an RN Case Manager.
