About The Position

The Field RN will be responsible for coordination and implementation of the overall plan of care for the assigned patients in various hospice settings to assure the best possible quality of life is attained. Responsibilities include providing all aspects of care for the terminally ill patient, providing patient and family education, completing all regulatory requirements of nurses for patients on Hospice, performing patient assessment and documentation, providing symptom management, developing a comprehensive Interdisciplinary Plan of Care, orienting new staff, and participating in Quality Improvement activities.

Requirements

  • Graduated from an accredited nursing program.
  • Valid Florida license to practice as a Registered Nurse.
  • Minimum 1 year experience in Medical Surgical nursing or equivalent.
  • Valid driving license and good driving record.
  • Ability to work within an interdisciplinary team framework.
  • Good oral and written communication skills.
  • Skilled in providing nursing care to pediatric, adolescent, adult, and geriatric patients.
  • Demonstrates good interpersonal skills, ability to cope with stress, maturity, and emotional stability.
  • Must meet criteria for employment by Halifax Health.

Nice To Haves

  • Hospice Experience preferred.
  • Certification in Hospice/Palliative preferred.

Responsibilities

  • Coordinate and implement the overall plan of care for assigned patients in hospice settings.
  • Provide all aspects of care for terminally ill patients.
  • Educate patients and families about care processes.
  • Complete all regulatory requirements for hospice nursing.
  • Perform patient assessments and documentation.
  • Manage symptoms effectively.
  • Develop a comprehensive Interdisciplinary Plan of Care.
  • Orient new staff members.
  • Participate in Quality Improvement activities.
  • Oversee care delivered by interdisciplinary team members.
  • Complete all aspects of the Hospice Intake & Admission process.
  • Implement and document changes to the interdisciplinary plan of care.
  • Maintain communication with physicians and other healthcare providers.
  • Identify educational needs and provide appropriate education to patients, families, and community partners.
  • Work as assigned to provide hospice services 24/7, including holidays.
  • Meet or exceed productivity standards.
  • Document efficiently in an electronic medical record (EMR).
  • Maintain company-owned equipment in good working order.
  • Attend required in-services and staff meetings.
  • Participate in Quality Action Teams and other QI activities.
