About The Position

Suite Living Hospice Care is dedicated to providing compassionate, high-quality hospice services to geriatric patients. Position Overview We are seeking a Hospice Case Manager to join our dedicated team. This role involves planning, organizing, and delivering hospice care with a strong emphasis on community health education. As a hospice RN, you will coordinate care for patients and their families, ensuring comfort, dignity, and quality of life.

Requirements

  • Graduate of an accredited School of Nursing
  • Active RN license in the state of Minnesota
  • CPR certification required
  • Minimum of 1 year of acute care experience (institutional setting) and 1 year of home care or hospice experience preferred
  • Strong clinical assessment, critical thinking, and problem-solving skills
  • Compassionate and patient-centered approach to care
  • Ability to lift, position, and transfer patients as needed
  • Must have a valid driver's license and insured vehicle for travel to patient locations

Responsibilities

  • Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice care needs.
  • Develop, implement, and adjust individualized care plans based on patient conditions and evolving needs.
  • Administer medications, treatments, and palliative care interventions as prescribed.
  • Educate patients and families on symptom management, medication use, and self-care techniques.
  • Provide emotional and psychological support to patients and their loved ones.
  • Assist with activities of daily living to enhance patient comfort and independence.
  • Serve as a Case Manager, coordinating care for assigned patients.
  • Maintain accurate, detailed clinical records and document changes in patient conditions.
  • Collaborate with physicians, caregivers, and interdisciplinary team members to ensure seamless care.
  • Participate in family and team meetings, providing updates and guidance on care plans.
  • Advocate for patient needs and facilitate access to necessary resources and support services.
  • Participate in on-call rotations as needed.
  • Ensure necessary medical equipment and supplies are available for patient care.
  • Supervise and support hospice aides as needed.
  • Engage in professional development and continuing education.
  • Contribute to quality improvement initiatives within the organization.
