About The Position

Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospice and Carroll Hospital Center. Provides professional, comprehensive, family-oriented nursing care to individuals in the home setting during the final stages of life. Assesses patients' and families' needs and formulates, implements and evaluates plan of care. Performs primary care case management function for assigned patients.

Requirements

• Associate's Degree from an accredited school of nursing.
• Minimum of two years of recent medical/surgical nursing experience.
• One year of home health experience preferred.
• Expertise in performing health assessments.
• CHPN - Certified Hospice Palliative Nurse.
• CPR - AHA Healthcare Provider certification.
• Professional licensure required in Maryland and Pennsylvania.

Nice To Haves

• Bachelor's Degree.
