And everyone, in their own unique way, plays an important part, every day. The Home Health & Hospice RN (HHHRN) provides care to patients who are in need of care in their homes. Home health & hospice nursing practice considers the needs of the patient, their family and/or the patient’s caregivers. The HHHRN provides care to patients across the lifespan, from prenatal to post-death periods. Home health nursing takes a holistic approach to personal health practices when treating diseases or disabilities. The practice embraces primary, secondary and tertiary prevention; assisting patients and families to coordinate community resources and health insurance benefits as well as hands on delivery of nursing services in the patient’s home.

Requirements

  • Associate degree in Nursing (ASN).
  • Current Massachusetts Registered Nurse (RN) license.
  • Current valid US-issued driver’s license and a registered, inspected, and insured automobile for work related purposes.
  • Ability to assess and respond to the needs of patients and families in varied settings, and cope with family emotional stress while respecting individual lifestyles.
  • Proficiency with Microsoft Office Products, and Internet.
  • High degree of confidentiality and able to handle situations in a diplomatic and tactful manner.
  • Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and/or governmental forms.
  • Ability to accurately write reports, business correspondence, procedures, and meet documentation requirements using computer technology.
  • Excellent communication skills, both written and oral.
  • Ability to effectively and professionally present information, communicate and interact, and respond to questions from managers, employees, customers, and the general public.
  • Ability to speak in ordinary conversational tones utilizing the English/American language and be able to understand ordinary conversations.
  • Ability to perform the following mathematical skills: multiply, divide, subtract, addition, and use equations.
  • General knowledge of statistical information, such as percentages, mean, mode and median and have the ability to interpret graphs and statistical reports.
  • Ability to identify and define problems, collect data, establish facts, and draw valid conclusions.

Nice To Haves

  • Bachelor’s degree in Nursing (BSN).
  • Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Responsibilities

  • Assumes primary responsibility for patient admissions to hospice care that includes the assessing, planning, implementing, coordinating, and evaluation of hospice care.
  • Evaluates referred patients for hospice care services and determines the types of services to be provided.
  • Ensures the appropriate care and services are initiated within the Hospice Medical Director and Attending Physician’s orders and the plan of care.
  • Completes Hospice Comprehensive assessment and Hospice Outcomes and Patient Evaluation (HOPE) assessment accurately and timely per policy and regulatory requirements.
  • Initiates communication with Hospice Medical Director and/or Attending Physician, other hospice care staff members, and other agencies as needed to coordinate optimal care and to use as a resource for the patient.
  • Adheres to and promotes the elements of the HealthAlliance Home Health and Hospice’s Compliance and HIPAA Programs.
  • Participates in Quality Assurance Performance Improvement (QAPI) activities.
  • Complies with established departmental policies, procedures, and objectives.
  • Attends variety of meetings, conferences, seminars as required or directed.
  • Demonstrates use of Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
  • Maintains regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.
