About The Position

AdvisaCare is expanding its Medicare Division and is seeking Home Care Nurses. This role offers a unique opportunity for nurses to provide one-on-one patient care, fostering real conversations and connections, unlike the fast-paced environment of traditional facilities. The position involves home care visits and shift work, focusing on a holistic and patient-driven approach. LPN Skilled Visits are also available, working under the supervision of a Registered Nurse. Nurses will utilize their knowledge and skills in a varied daily routine, making a significant difference in patient health and safety, including specialized wound care and patient education.

Requirements

• Current Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN)
• Updated Tuberculin (TB) test
• Current CPR certification
• Clear Criminal History/Drug Screen
• Compassionate and caring demeanor, with a professional presentation at all times
• Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home)
• Critical thinking, task competence and the ability to think on your feet
• Demonstrated knowledge and skills to provide care to and communicate with all ages of the population

Nice To Haves

• Minimum one (1) year of Home Health nursing experience preferred
• OASIS Experienced Registered Nurse preferred

Responsibilities

• Provide wound care, including Wound Vacs, Drains, Tubes, All Ostomies, Traches, Catheters, Infusions, and LVADs
• Get to know patients on a personal level
• Provide one-on-one education to patients and their caregivers
• Coordinate with all members of the Home Health Team
