About The Position

Requirements

  • Active Massachusetts RN license
  • Successful completion of required pre-hire medication assessment
  • Minimum 1 year of nursing experience , or 6 months with a BSN
  • CPR certification required
  • Strong computer and documentation skills
  • Travelers only

Responsibilities

  • Patient Assessment & Care Planning Conduct comprehensive admission assessments, including physiological, psychosocial, environmental, and health history evaluations.
  • Identify patient problems and develop individualized plans of care.
  • Initiate, implement, revise, and document care plans based on ongoing patient needs.
  • Clinical Outcomes & Case Management Maintain Home Health Resource Group (HHRG) values at national standards.
  • Utilize HHRG data to determine visit frequency and interdisciplinary resource utilization.
  • Ensure patient outcomes consistently meet or exceed national benchmarks.
  • Coordination & Collaboration Participate in interdisciplinary case conferences to evaluate and adjust plans of care.
  • Communicate significant changes in patient condition to physicians and care team members.
  • Refer patients to appropriate community resources when indicated.
  • Supervision & Education Provide instruction and documented supervision of Home Health Aide care plans every 14 days.
  • Deliver patient and caregiver education and document understanding and response.
  • Documentation & Compliance Complete all admissions, visit notes, and required documentation accurately and on time.
  • Maintain detailed narrative documentation reflecting skilled nursing care.
  • Obtain, verify, and clarify valid physician orders for all services rendered.
  • Discharge Planning Coordinate and plan appropriate patient discharges.
  • Communicate discharge plans with patients and physicians.
  • Complete all required discharge documentation and summaries.
  • Perform additional duties as assigned.
