About The Position

AdvisaCare is a fast-paced, established Home Health / Medicare Agency seeking FULL TIME Skilled Registered Nurses (RN) to provide care and service to patients in the Greater Portage (Kalamazoo County) and surrounding areas. The role emphasizes one-on-one patient care, allowing nurses to build real connections and provide holistic, patient-driven care. The position involves home care visits and shift work, focusing on a single patient at a time, unlike traditional facility settings. AdvisaCare also accepts part-time nurses willing to complete a minimum of 30 visits per month. LPN Skilled Visits are also available, working under the RN. The role involves diverse tasks such as Wound Care (including Wound Vacs, Drains, Tubes, All Ostomies, Traches, Catheters, Infusions, and LVADs), patient and caregiver education, and coordination with the Home Health Team. Training is provided to enhance necessary skills.

Requirements

  • Current Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Demonstrated knowledge and skills to provide care to and communicate with all ages of the population
  • Updated Tuberculin (TB) test
  • Current CPR certification
  • Clear Criminal History/Drug Screen
  • Compassionate and caring demeanor, with a professional presentation at all times
  • Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home)
  • Critical thinking, task competence and the ability to think on your feet is needed

Nice To Haves

  • OASIS Experienced Registered Nurse preferred
  • Minimum one (1) year of Home Health nursing experience preferred

Responsibilities

  • Provide care and service to patients in the Greater Portage (Kalamazoo County) and surrounding areas
  • Conduct home care visits
  • Perform shift work
  • Provide one-on-one patient care
  • Perform Wound Care, including Wound Vacs, Drains, Tubes, All Ostomies, Traches, Catheters, Infusions and even LVADs
  • Provide one-on-one education with patients and their caregivers
  • Coordinate with all members of the Home Health Team
