Registered Nurse - Home Health
About The Position
Klarus Home Health is hiring dedicated and compassionate Registered Nurse Case Managers to provide nursing services and case management to our patients who receive home health services in the San Antonio, TX and surrounding areas. The registered nurse plans, organizes and directs home health care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Requirements
- Current State or Compact Registered Nurse (RN) License in hand
- A minimum of one year clinical experience is preferred
- Demonstrate knowledge and skillset in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
- HCHB EMR knowledge is desired
Responsibilities
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
