Requirements

  • Current, unrestricted Registered Nurse ( RN) license in the state of practice
  • 1+ year of clinical nursing experience
  • Ability and willingness to travel within the assigned coverage area
  • Ability to lift, push, pull, or support up to 50 pounds , including assisting with patient positioning or transfers
  • Valid driver’s license, reliable vehicle, and current auto insurance per state requirements
  • Associate or Bachelor’s Degree in Nursing ( BSN preferred)
  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Nice To Haves

  • Home Health or Hospice experience preferred
  • Experience with HomeCare HomeBase ( HCHB) preferred

Responsibilities

  • Conduct comprehensive assessments of patients and families to identify physical, psychosocial, and environmental needs
  • Develop, implement, and manage individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Perform initial evaluations and ongoing reassessments to monitor patient progress
  • Provide skilled nursing care and education to patients and caregivers
  • Communicate significant findings, changes in condition, and safety concerns with physicians, clinical leadership, and care team members
  • Update and revise care plans based on patient needs and clinical outcomes
  • Maintain accurate and timely documentation in accordance with regulatory and organizational standards
  • Ensure strict compliance with HIPAA privacy regulations and patient confidentiality
  • Travel within the service area to provide care in patients’ homes
