About The Position

The Registered Nurse is responsible for providing safe and effective delivery of patient care through assessment, planning, implementation and evaluation of patient and family needs in the home care environment. Directs and supervises the home health aide in providing quality patient care services. The RN is responsible for organizing and managing cases including coordination with other professional disciplines (PT-OT-ST-MSW).

Requirements

Graduate of an accredited school of nursing.
Licensed in the state of Illinois.
BLS certification required.
Two years of experience in a health care setting is required.
Driving and reliable transportation is a necessity.

Nice To Haves

Home health experience is preferred.

Effectively utilizes nursing process to ensure optimal nursing care and patient outcomes.
Provides age –appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to primarily adult and geriatric populations as indicated.
Assesses patient and family learning needs and provides ongoing education and evaluation.
Adheres to standard precautions and infection control standards.
Documents readiness to learn, teaching, barriers and outcomes.
Reassesses significant change in patient’s condition and updates care plan appropriately.
Identifies patient care needs to formulate and/or contribute to the discharge plan.
Completes documentation accurately and in a timely manner, according to policies and procedures.
