About The Position

The Registered Nurse-Home Health, in conjunction with the Case Manager, utilizes physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as agency policies and procedures. The Registered Nurse-Home Health is responsible for planning, coordinating and documenting care for an assigned caseload of patients. Participates in Quality Assessment and Performance Improvement (QAPI) activities. This is a full-time position, 40 hours/week, including on-call rotation, located at St. Anthony’s Hospital in Springfield, IL, serving a specific service area. Requirements

  • Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice
  • Valid drivers license
  • Automobile insurance
  • Own transportation to make home visits
  • Licensed as a Registered Nurse (RN) in the state of practice
  • Basic Life Support (BLS)

Nice To Haves

  • Bachelor's Degree in nursing is preferred
  • Prior experience in home health or hospice is preferred
  • 1 year acute nursing experience is preferred
