Requirements
- Must be at least 21 years of age.
- A valid non-restricted RN license in the state of Colorado is required.
- Must have a valid non-restricted driver’s license.
- Flexibility and availability to work a weekend schedule if needed.
Nice To Haves
- At least 1 year of home health or hospice experience preferred.
Responsibilities
- Plans, coordinates, implements, and evaluates the nursing care of all patients.
- Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
- Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
