Requirements

  • Must be at least 21 years of age.
  • A valid non-restricted RN license in the state of Colorado is required.
  • Must have a valid non-restricted driver’s license.
  • Flexibility and availability to work a weekend schedule if needed.

Nice To Haves

  • At least 1 year of home health or hospice experience preferred.

Responsibilities

  • Plans, coordinates, implements, and evaluates the nursing care of all patients.
  • Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
  • Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
