About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) for a 13-week Home Health contract assignment in South Dennis, MA. The RN will provide high-quality, patient-centered care in a home setting, ensuring individualized treatment plans are implemented effectively while promoting patient independence and well-being. This role requires strong clinical assessment skills, excellent communication, and the ability to work independently in a community-based environment.

Requirements

• Current and active Registered Nurse (RN) license in Massachusetts.
• Minimum of 1 year of nursing experience required OR 6 months of experience with a BSN.
• Current CPR certification required.
• Successful completion of required pre-hire medication competency testing.
• Strong computer and documentation skills.

Nice To Haves

• Home health experience preferred.
• IV therapy experience preferred.
• BSN preferred.

Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans.
• Perform comprehensive patient assessments and ongoing evaluations.
• Administer medications, including IV therapy when required.
• Educate patients and families on disease management, medications, and care plans.
• Document patient care accurately using electronic medical records.
• Collaborate with interdisciplinary team members to coordinate care.
• Ensure compliance with state, federal, and agency regulations.
