About The Position
The Home Care RN delivers individualized nursing care to patients in their homes, helping them maintain health, recover from illness, and manage ongoing conditions. This role requires strong clinical skills, independence, and a commitment to patient-centered care.
Requirements
- Active Registered Nurse (RN) license
- Degree in nursing from an accredited institution
- Current CPR/BLS certification
- Clinical experience required
- Strong assessment, organizational, and critical thinking skills
- Ability to work autonomously and manage multiple patient visits
- Reliable transportation and willingness to travel locally

Responsibilities
- Provide in-home nursing assessments and develop patient-specific care plans
- Deliver skilled nursing interventions, including medication administration, wound care, and health monitoring
- Evaluate patient progress and modify care plans as needed
- Document all care activities in a timely and accurate manner
- Instruct patients and caregivers on disease processes, treatments, and home safety
- Collaborate with physicians and other healthcare professionals to coordinate care
- Identify and address potential health or safety concerns in the home
