Requirements

• Be currently licensed as a Registered Nurse (RN) in the state of Idaho.
• A minimum of one year of clinical experience is preferred but not required.
• Demonstrate knowledge and skill in nursing practice.
• CPR professional certification.
• General knowledge of computers and the ability to learn industry-specific software and documentation applications.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Work with patients, their families, and other healthcare professionals in the patient’s place of residence.
• Perform visits and complete coordination of care.
• Act as the point of contact for all disciplines involved with providing care to the patients.
• Follow agency, state, and federal policies and procedures.
• Consult with the patient’s primary care physician.
• Collect OASIS information (Home Health only).
• Ensure medication profiles are accurate.
• Attend meetings as required.
