About The Position

The position is for a part-time Registered Nurse (RN) at Advocate Health, specifically within the Home Health Nursing department. The role is PRN (as needed) and involves collecting and analyzing data related to healthcare consumers' health, developing individualized care plans, implementing care strategies, and evaluating patient outcomes. The RN will also delegate care tasks to appropriate healthcare workers and must be able to respond quickly to changes in patient conditions. Physical requirements include the ability to lift and support patients and medical equipment, as well as perform various physical tasks.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state of work and residence.
  • BLS required per policy guidelines.
  • Additional education, training, certifications, or experience may be required within the department.

Nice To Haves

  • BSN preferred.
  • Experience in home health nursing.

Responsibilities

  • Collect data pertinent to the healthcare consumer's health or the situation.
  • Analyze assessment data to determine diagnoses or issues.
  • Identify expected outcomes for a plan individualized to the healthcare consumer or the situation.
  • Develop a plan that prescribes strategies and alternatives to attain expected outcomes.
  • Implement the identified plan, coordinate care delivery, and employ strategies to promote health and a safe environment.
  • Evaluate progress toward attainment of outcomes.
  • Delegate elements of care to appropriate healthcare workers in accordance with applicable legal or policy parameters.
