About The Position

Park Nicollet is looking to hire a Registered Nurse to join our Home Care/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day. Position Summary: The Home Care/Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. Work Schedule: M-F (with 1 regular day off per week), every 6th-7th weekend. Monthly on-call, bi-annual weekend on-call. 2 Holidays/year. Location: Central Team – Mpls/SLP

Requirements

  • Work experience as a RN for at least two years
  • Current MN Registered Nursing License
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
  • Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record.

Nice To Haves

  • Experience in home care and or hospice desirable.
  • Must be able to function independently with minimal supervision.
  • High level of sensitivity, assessment skills, technical-skills and communications skills.
  • Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
