Responsibilities include completing comprehensive assessments (including OASIS when required), developing and updating individualized plans of care, and providing skilled nursing services across preventative, rehabilitative, palliative, and end-of-life needs. Educates and counsels patients and families on disease processes, symptom management, and self-care, and coordinates care with interdisciplinary team members such as Home Health Aides, Therapies, Social Work, Chaplaincy, and Bereavement Services. This position also supervises LPNs and paraprofessionals, communicates changes in patient condition to physicians and team members, secures necessary equipment and supplies, and maintains thorough, timely documentation within the electronic medical record.

Requirements

Licensed RN in the State of Michigan
Able to meet Home Health and Medicare standards through criminal background check and fingerprinting.
Possess current Michigan motor vehicle license, ability to drive a car and an insured vehicle capable of transport to patient homes or other offices in various weather conditions.
Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.

Nice To Haves

Experience in acute care, home care, or hospice preferred
Proven health assessment, organizational, and interpersonal skills with the ability to make independent judgments directing patient care.
