Home Based Primary Care (HBPC) Patient Aligned Care Team (PACT) Staff Registered Nurse (RN) is responsible for providing competent, evidence-based care to Veterans enrolled into HBPC program. This nurse provides direct patient care and care coordination, as it pertains to Veterans primary care needs. Responsible for case management, assessment, triage, education, resource coordination, medication reconciliation, treatment coordination, and care planning. This nurse plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, disease prevention, geriatric care, primary care, and palliative care. Utilizes interventions, to maximize health capabilities and reduce unnecessary urgent care visits, hospital admissions, and emergency room visits, per Veteran's goals of care. Performs home visits, along with video and telephone visits, when appropriate. Also assist in directing the provision of nursing education, orientation, competencies, providing quality improvement and outcomes utilization. Demonstrates leadership in delivering and improving holistic, home-based care through collaborative strategies with others.
