About The Position

The HEDIS Registered Nurse at Walker Healthforce is responsible for leading a multidisciplinary team to deliver a holistic, person-centric care management program for a diverse health plan population. This remote contract position involves serving as the primary contact for members, caregivers, and providers, utilizing various communication channels to ensure effective care coordination. The role requires the use of case management processes to assess, develop, implement, monitor, and evaluate care plans aimed at optimizing member health across the care continuum.

Requirements

• 2+ years nursing experience
• TN RN License
• Knowledge of Windows and Microsoft Office, specifically Excel.

• Experience in chart review/chart abstraction.
