About The Position

The HEDIS Registered Nurse at Walker Healthforce is responsible for leading a multidisciplinary team to deliver a comprehensive care management program tailored to a diverse health plan population. This role involves serving as the primary contact for members, caregivers, and providers, utilizing various communication methods to ensure effective care coordination. The RN will employ the case management process to assess, develop, implement, monitor, and evaluate care plans aimed at optimizing member health across the care continuum.

Requirements

• 2+ years nursing experience
• TN RN License
• Knowledge of Windows and Microsoft Office, specifically Excel.

• Regional travel experience
• Experience in chart review/chart abstraction.
