About The Position

The transplant coordinator will join a dynamic multidisciplinary team that currently performs 1% of heart transplants worldwide. The role primarily involves outpatient care to patients who undergo cardiac transplantation. The scope of the position includes patient care, education, quality assurance, research, administrative activities. The transplant coordinator works in close collaboration with advanced heart failure cardiologists, cardiac surgeons, social workers, pharmacists, financial coordinators, psychiatrists, etc.

Requirements

  • Relevant nursing experience in cardiology and/or transplant strongly preferred.
  • Exceptional time management and organizational abilities when providing care.
  • Ability to identify patient/family learning needs and implement appropriate programs.
  • Expertise working with multidisciplinary departments.
  • Proficiency with electronic data collection and documentation.
  • Bachelor’s degree in nursing or equivalent experience required.
  • Current Massachusetts licensure as a registered nurse required.
  • Bachelor's Degree Nursing required and Master's Degree Nursing required
  • Registered Nurse [RN - State License] - Generic - HR Only preferred
  • Clinical Nurse Specialist [CNS - State License] - Generic - HR Only preferred
  • Clinical nursing experience 3-5 years required
  • In-depth knowledge of clinical nursing care practices and principles.
  • Strong clinical assessment and critical thinking skills.
  • Proficiency in developing and implementing evidence-based care plans.
  • Excellent communication and interpersonal skills to collaborate effectively with interdisciplinary teams, patients, and families.
  • Leadership abilities to provide guidance, mentorship, and education to nursing staff.
  • Ability to stay updated on the latest advancements in nursing skills and patient care through continuous learning and professional development.

Nice To Haves

  • Clinical education experience 2-3 years preferred
  • Nursing leadership experience 1-2 years preferred

Responsibilities

  • Coordinate all aspects of the transplant process, including patient assessments, and care delivery throughout the transplant journey.
  • Engage in communication with patients via phone calls, portal messages, and direct contact and collaborate with advanced practice providers (APPs) and transplant physicians in care delivery.
  • Provide and maintain comprehensive and accurate documentation in the patient medical record.
  • Initiate, facilitate and develop interdisciplinary collaboration.
  • Participate in planning, designing, implementing, or evaluating guidelines, protocols, and standards.
  • Assist with data collection and documentation that is reported to the New England Donor Services (NEDS) & the United Network for Organ Sharing (UNOS).
  • Identify patient/family learning needs and develop and implement programs to meet those needs.
  • Initiate, participate and contribute to educational programs for staff at the unit/service and interdepartmental levels.
  • Collaborate with other transplant programs to ensure efficient and effective use of resources and expertise.
  • Participate in the Multidisciplinary Cardiac Transplant Selection meeting to determine patient candidacy for cardiac transplantation.
  • Initiate and participate in quality assurance activities.
  • Participate in and support research activities.
  • Provides expert-level consultation and advises clinical staff on the formulation of diagnoses and treatment plans for patients and families.
  • Plans and delivers relevant staff and patient education to nurses and other members of the health care team on matters regarding plan of care for patient and family.
  • Actively participates in research and evidence-based practice activities that will positively affect patient care outcomes.
  • Provide health care services to patients.
  • Forming a diagnosis and management and treatment of illness and disease.
