About The Position

This position is for a RN Supervisory Program Director for Home Based Primary Care and is located in Murfreesboro Tennessee. This RN is primarily responsible and accountable for the management and supervision evaluation of care for patients enrolled in TVHS Home Based Primary Care Program. Home-Based Primary Care (HBPC) Program director is responsible and accountable for all elements for the management, supervision and evaluation of care for patients enrolled in the TVHS HBPC program at the Nashville and York sites, with potential oversight of other future HBPC locations within TVHS. The HBPC Program Director is responsible for directing the clinical and administrative services offered by the program to ensure that the compliance with local and VHA standards and policies, as well as accreditation standards for home care organizations. He/she is responsible for the development and continued effective functioning of the interdisciplinary health care team. He/she recognizes and utilizes the skills, knowledge and contributions of each team member. He/she collaborates with the HBPC Medical Director on daily program management and collaborates with other HBPC program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis. Demonstrates expertise in professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) with improved evidence-based and/or data-driven outcomes beyond the immediate practice setting. Responsibilities
