About The Position

The Registered Nurse (RN) will serve as a Care Coordinator for veterans assigned to the Geriatric Patient Aligned Care Team (PACT) at the Department of Veterans Affairs. This role involves performing clinical and administrative duties, assessing and triaging patients, and ensuring comprehensive documentation in medical records. The RN will coordinate care for veterans with chronic diseases, complex medical needs, and those requiring intensive continuing care, working collaboratively with an interdisciplinary team to develop treatment plans and monitor patient status.

Requirements

• U.S. Citizenship is required.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
• Graduation from a school of professional nursing approved by the appropriate accrediting agency.
• Proficiency in basic written and spoken English.

Nice To Haves

• Three years of nursing experience.
• Experience in Primary Care/PACT.
• Triage/Telephone Triage experience.
• BLS certification.

Responsibilities

• Provide care coordination for veterans in the Geriatric Patient Aligned Care Team (PACT).
• Assess functional status, acuity level, and treatment needs of veterans.
• Initiate contact with veterans and caregivers to ensure optimal clinical outcomes.
• Monitor veterans' status and health service usage during care.
• Collaborate with the interdisciplinary team to develop and coordinate treatment plans.
• Coordinate performance improvement initiatives and collect data for accreditation standards.
