Registered Nurse Geriatric Care Manager

Diversified Life Care Partners is seeking a highly experienced Registered Nurse with extensive geriatric expertise and leadership experience in senior care. This is a senior-level role designed for a nurse who understands the complexity of aging — medically, functionally, and systemically — and who is ready to practice with autonomy, depth, and purpose. Requirements
- 10–20+ years of nursing experience with significant focus on the geriatric population
- Leadership or management experience in senior living, hospice, home health, hospital case management, or care management strongly preferred
- Demonstrated ability to independently assess risk and make sound clinical judgments
- Experience supervising staff or overseeing care plans preferred
- Excellent communication and systems-thinking skills
- Ability to travel throughout the Lexington region (reliable transportation required)
- Comfort with electronic documentation and mobile scheduling tools
Responsibilities
- Conduct comprehensive, in-home geriatric nursing assessments
- Identify medical, cognitive, medication, safety, and functional risks
- Develop and oversee individualized, whole-person care plans
- Provide clinical oversight and accountability for services in the home or senior living settings
- Advocate for clients during physician visits, hospitalizations, and care transitions
- Guide families through complex decisions with clarity and confidence
- Collaborate with physicians, attorneys, fiduciaries, financial planners, and community partners
- Document assessments, recommendations, and outcomes in a secure care management platform
- Function as a clinical leader and trusted advisor
