Diversified Life Care Partners is seeking a highly experienced Registered Nurse with extensive geriatric expertise and leadership experience in senior care. This is a senior-level role designed for a nurse who understands the complexity of aging — medically, functionally, and systemically — and who is ready to practice with autonomy, depth, and purpose. We are looking for a nurse who has supervised teams, overseen care delivery, managed complex cases, or served in leadership within senior living, hospice, home health, hospital case management, or professional care management. Requirements

  • 10–20+ years of nursing experience with significant focus on the geriatric population
  • Leadership or management experience in senior living, hospice, home health, hospital case management, or care management strongly preferred
  • Demonstrated ability to independently assess risk and make sound clinical judgments
  • Experience supervising staff or overseeing care plans preferred
  • Excellent communication and systems-thinking skills
  • Ability to travel throughout the Lexington region (reliable transportation required)
  • Comfort with electronic documentation and mobile scheduling tools

Responsibilities

  • Conduct comprehensive, in-home geriatric nursing assessments
  • Identify medical, cognitive, medication, safety, and functional risks
  • Develop and oversee individualized, whole-person care plans
  • Provide clinical oversight and accountability for services in the home or senior living settings
  • Advocate for clients during physician visits, hospitalizations, and care transitions
  • Guide families through complex decisions with clarity and confidence
  • Collaborate with physicians, attorneys, fiduciaries, financial planners, and community partners
  • Document assessments, recommendations, and outcomes in a secure care management platform
  • Function as a clinical leader and trusted advisor
