Requirements

Valid and current Registered Nurse (RN) license to practice in the state.
Minimum of 2 years experience in nursing, preferably in home health care or private duty nursing.
Strong clinical skills including medication administration, wound care, and patient assessment.
Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members.
Ability to work independently with strong critical thinking and problem-solving abilities.
Compassionate, patient-focused attitude with a commitment to providing high-quality care.
Basic computer skills for documentation and communication purposes.

Responsibilities

Provide comprehensive nursing care to patients in their homes including medication administration, wound care, and health monitoring.
Develop and implement individualized care plans based on patient assessment and medical history.
Collaborate with physicians, therapists, and family members to coordinate patient care and communicate progress.
Monitor patient condition and report any changes or concerns promptly to the appropriate healthcare providers.
Maintain accurate and detailed patient records in compliance with legal and company standards.
Educate patients and their families on disease management, rehabilitation exercises, and health maintenance.
Ensure adherence to infection control, safety protocols, and ethical standards throughout all care activities.
