About The Position

This role will be working in Multiple locations and will primarily float to the following clinics: (Role will float to St Paul Specialty Center 401 and St Paul Wabasha HealthPartners clinics.). Could include other locations in the future. The RN position will support Endocrinology, Nephrology and Rheumatology. POSITION PURPOSE: Service excellence is to be centered on patient care and patient relationships and is the responsibility of all employees. Teamwork is the norm and all employees will be held accountable to work as effective multidisciplinary care team members. Effectively provides high quality service to both internal and external customers Provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition. Delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity Creates and implements dynamic teaching plans based upon identified patient need Acts as a resource for other staff

Requirements

  • Graduate of an accredited school of nursing
  • Current RN licensure in Minnesota
  • Current RN licensure in Wisconsin within two months of hire
  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable
  • Strongly demonstrated written and oral communication skills
  • Ability to sit or stand for prolonged periods at a time.
  • Must possess speech, hearing and sight skills to perform the functions of the position.
  • Manual dexterity required.
  • Must be able to speak, read and write English proficiently.

Responsibilities

  • Demonstrates clinical competence in using the nursing process to provide care to patients and their families.
  • Implements established Nursing Policies and Procedures and Standing Orders.
  • Provides emotional, intellectual and psychological support to patients and their families.
  • Documents accurately and completely and keeps record regarding clinical practice.
  • Shares information with other members of the health care team.
  • Makes appropriate patient referrals.
  • Uses interdisciplinary resources to enhance own clinical decision making Provides continuity among home, office, inpatient setting and community.
  • Uses appropriate systems, automated or manual, to obtain patient information or schedule appointments.
  • Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans
  • Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care
  • Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention
  • Identifies and secures patient services needed for patient care
  • Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.
  • Works in partnership with patient/family to identify patient and family needs and develop a plan for education and/or make appropriate nursing referrals.
  • Teaches techniques or knowledge necessary for prevention, self-care management or rehabilitation.
  • Engages in ongoing evaluation of the effectiveness of teaching in terms of understanding and application.
  • Participates in patient care conferences, in-services and educational programs.
  • Keeps current on best practices related to nursing and patient care
  • Serves as a role model in the enhancements of nursing practice and professional conduct.
  • Demonstrates flexibility in performing additional responsibilities to contribute to the improvements in patient care and the functioning of the care unit and other support services.
  • Provides support to professional and technical staff.
